    • Please bring all copies and/or radiologist reports

    • If you don’t have a copy of the imaging (XR, CT, MRI, Ultrasound), please contact us beforehand to ensure we can access your images electronically

  • Relevant blood tests

  • Relevant correspondence from other healthcare professionals

  • Please complete the New Patient Form Online here, or in person 10-15minutes prior to your appointment.

How do I book an appointment with Kade?

Please call us on (03) 9124 9812 to make an appointment. Please have a referral letter from your general practitioner prior to the appointment.

Is there patient parking at Kade’s Rooms?

Parking is available at the Hawthorn East rooms (off-street and on-street), and parking available at the hospital consulting rooms.

Do I need a referral to see Kade?

Yes, a referral is needed to be eligible for a medicare rebate.

Do I need to pay for my consultation at the time of my appointment?

Yes, payment is accepted in EFTPOS or credit card only. Cash or cheques are not accepted.

Does Kade Bulk Bill?

No, patients wishing to see Dr Collins & be bulk billed can be referred to the Orthopaedic Department at Peninsula Health (Frankston Hospital).

Will my health insurance cover my surgery/procedure?

Informed financial consent will be given prior to any surgery to avoid any surprises in costs, including an MBS item number which you can check with your health insurer if you’re covered.

Costs of surgery that may not be covered by your private health insurance can include surgeon fees, surgical assistant fees, anaesthetist fees, hospital bed, theatre and equipment costs, pathology (blood tests), radiology, and pharmacy costs.

Common item numbers are: total hip replacement (49318), total knee replacement (49518), partial knee replacement (49517), knee arthroscopy and meniscal surgery (49572, 49580, 49582), ACL reconstruction (49542).

Does Kade charge a gap fee for surgery?

Kade is a no or known-gap provider for most private healthcare insurers. Kade is fortunate to be able to offer no-gap surgery for appropriately insured patients at:

  • Adeney Private Hospital (Medibank Private and BUPA)

  • Mulgrave Private Hospital (Medibank Private)

What is informed financial consent?

Yes.

Do you see Worksafe / TAC / Veteran Affairs (DVS) patients?

Yes.

Do I need Xrays or MRI scan prior to my appointment?

  • Please bring along any relevant imaging or tests you have had.

  • If you only have electronic copies of the imaging, please ensure you let Kade’s rooms know prior so we can ensure we have access to the images during your consultation.

Does Kade treat children?

Yes, Kade is happy to treat children’s injuries/fractures.
